Skin rejuvenation today encompasses a host of different types of treatments – from “SkinPen®” to Ultherapy Uplifting Ultrasound.  The most popular, though, this time of year is skin enhancing laser resurfacing.

The fall and winter months are ideal for this type of procedure, as patients are encouraged to refrain from sun exposure both before and after treatments.  The new skin exposed following treatment is particularly vulnerable to the sun’s harsh rays – something that is less concerning heading into fall and winter, when sun intensity and outdoor activity are reduced.

Laser resurfacing treatments target:

  • Sun spots and damage
  • Wrinkles
  • Photo-aging
  • Solar lentigines (pigmented flat or slightly raised lesions that, unlike freckles [ephilis], do not fade in the winter months)
  • Facial telangiectasia (condition causing dilation of the capillaries, which appears as small red or purple clusters)
  • Vascular and pigment irregularities

The most popular areas for skin resurfacing include the face, neck and chest. This type of skin rejuvenation can dramatically enhance the youthful appearance, brightness and texture of the skin – while encouraging the new development of collagen.

Christie Brinkley, the 62-year-old ageless beauty and lifelong model, revealed in a recent interview that while BOTOX® and dermal fillers are a part of her beauty routine, she believes that laser treatments are “miraculous” (New Beauty magazine, Fall-Winter 2016).

The two primary categories into which laser treatments fall include ablative (removes thin layers of skin, prompts collagen growth) and non-ablative (subtly stimulates collagen growth).

Ablative Skin Resurfacing

Ablative skin resurfacing directs short, concentrated pulsating beams of light over the skin, vaporizing skin cells damaged at the surface level and precisely removing skin layer by layer.  The intense heat from the laser causes the right amount of controlled damage to the target area, stimulating the underlying skin and prompting collagen formation.  New smother, tighter skin then forms.

This procedure requires a topical anesthetic, which is applied 30 to 90 minutes prior to treatment.  More intense treatments may also include a mild sedative.  Patients wear protective eye shields during the procedure and receive an anesthetic ointment following the procedure to reduce discomfort.

Non-ablative Skin Resurfacing

The fractional lasers used in non-ablative skin resurfacing use lower energy levels than ablative lasers. During the procedure, a laser head delivering heat into the skin through thousands of tiny, deep columns known as microthermal treatment zones is passed over the treatment area in a series of horizontal and vertical overlapping passes.  Patients may experience a slight prickling or burning sensation during the process.  The heat from the laser promotes collagen production and reduces the appearance of fine lines, though the results are less dramatic as ablative skin resurfacing and multiple treatments may be required.

Recovery

Aftercare instructions will vary depending on the type of resurfacing procedure you undergo, though protecting the treated skin from the sun is underscored for all.

Recovery for a non-ablative skin resurfacing treatment will be more rapid than an ablative skin resurfacing procedure, which may require two to four weeks to peel and begin to heal.

The best type of laser treatment and number of treatments required will be decided following a thorough consultation with an experienced professional and assessment of the target area(s) as well as overall patient health.  Ablative treatments, in particular, should be administered by a registered nurse experienced with such laser treatments and technology.

Our Faces Are Not Symmetrical from Left to Right

When you look at someone’s face, you might not notice it right away, but if you look closely, you’ll notice that their face isn’t symmetrical. This is common. Our faces, and bodies, as well, are typically asymmetrical to some degree. We see a lot of faces at DermaTouch RN, and it’s the extremely rare occasion that someone comes in with an almost perfectly symmetrical face. In fact, some studies show that we prefer a bit of asymmetry because it adds character and distinction to our face. Even models, who are said to have symmetrical faces, often have asymmetrical faces if you look close enough (and before they’re photoshopped!).

Virtually every feature on our face is asymmetrical.

From our eyebrows to our lips to our nasolabial folds, every feature of our face on one side sits slightly higher or lower, to the left or right, or is deeper than the same feature on the other side. For example, one eyebrow will be higher than the other or might sit out a bit more on the face; the lips might sit slightly more to the left or right rather than in the center of the face directly under the nose; one nasolabial fold will typically be deeper than the other, and so on.  In other words, we should think of features such as eyebrows and cheekbones as sisters and not twins.

It is true that some may have had an accident or something happened to where the asymmetry is more noticeable, but generally, we don’t take a second look at someone because we “notice” the asymmetry.

Many seek out MedSpas to “fix” asymmetry.

Many seek out the services of a MedSpa to create more symmetry in the face. Though this is possible to a degree with treatments that include fillers, like Juvederm, Voluma, and Restylane, or Botox, it’s not the perfect fix. Fillers are going to settle in with your current bone structure and features, so there will still be some asymmetry, regardless of what you’re promised. This is important to remember. Fillers can help you achieve more symmetry—and we love helping our clients see that—but there will still be at least a little asymmetry when it’s all said and done. In reality, this is a good thing, because it will give you a more natural look without the “fake” look we strive to avoid. Asymmetry also means that one side of your face will need more filler than the other to reach more symmetry.

Legal vs. Illegal Practices – Do You Know Who is Injecting You?

When you enter a MedSpa or cosmetic service provider’s office, you expect to receive treatment from a professional, licensed individual who knows what he or she is doing. Unfortunately, this isn’t always the case. With the increase in “17032306 - legal vs illegal illustration design over a blackboardspa” parties, medispas, and other establishments boasting to offer cosmetic services such as Botox and fillers, it’s important to do your homework to ensure you’re working with an establishment that’s legally performing the services you seek. Did you know that all injectables require a prescription?

When we speak of injectibles, we mean fillers such as Juvederm products (Voluma, Ultra, Ultra Plus), Restylane, Kybella, Sculptra, Radiesse, Botox, and Dysport. These all require a prescription before being injected. Further, certain chemical peels require a prescription.

Who can legally inject with a prescription?

Per our recent blog post, “Who Can Legally Inject Based on Texas Medical Board Rule 197.13?” we shared that, “Per the Texas RULE §193.17, a midlevel practitioner, in addition to a physician, can inject with a written order. A midlevel practitioner is defined as “a physician assistant or advanced practice registered nurse.”

Who should NOT be injecting you?

Licensed Estheticians of any sort are not permitted to inject or use any product or procedure that comes in contact with or penetrates the dermis layer of the skin. In other words, it is illegal for an esthetician to be injecting you, even if delegated by a doctor. Unfortunately, this type of practice happens more often than it should.  Also, an esthetician is not authorized to use lasers for hair removal, teeth whitening, or skin resurfacing.

Medical assistants who are injecting offer another area of concern. “Medical assistant” has no true legal significance in Texas. There are no required training, licensing, or certifications. When direct care is provided by a medical assistant, the patient must be notified of such. Medical assistants are not permitted to write prescriptions and in most cases are prohibited from doing cosmetic laser treatments, fillers, or Botox injections without a prescription. Doctors may be subject to disciplinary action for professional misconduct if they allow a medical assistant to do such things.

How can we ensure legal vs. illegal practices?

Always do your research before having any type of cosmetic treatment performed. Ask for licenses and credentials.
